Job Summary
Conduct home health visits in West and South Philadelphia areas, administering medications, wound care, and treatments per designated care plans. Perform client assessments, monitor conditions, and report changes to managers while accurately documenting observations and interventions on a touch pad tablet. Execute and follow up on doctors' orders, manage case coordination, and travel to assigned cases as needed. Requires a current Pennsylvania RN license with one year of recent clinical experience.
Required Qualifications
- A current license as a Registered Nurse in Pennsylvania
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
