Job Summary
Conduct head-to-toe assessments, triage urgent clinical situations independently, and manage acute and post-acute care for clients in home settings. Perform comprehensive wound assessments, medication reconciliation, patient teaching, and safety monitoring while maintaining accurate point-of-care documentation. Manage a dynamic visit schedule with flexibility to adapt to changing patient needs and varied home environments. Requires a current Indiana nursing license, one year of recent hands-on experience in acute care or home health, and the ability to make real-time clinical decisions without immediate support.
Required Qualifications
- A current nursing license in good standing in the state of Indiana
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med surg, sub-acute care or rehab units (medical or skilled nursing settings)
