Job Summary
Conduct home health visits throughout Lebanon County, administering medications, wound care, and treatments per designated care plans. Monitor client conditions, report changes to managers, and execute doctor's orders while documenting observations and interventions via a touch pad tablet. Perform client assessments, manage case coordination, and maintain accurate records of evaluations and services provided. Requires a current Pennsylvania RN license with one year of recent clinical experience and ability to travel to assigned cases.
Required Qualifications
- A current license as a Registered Nurse in Pennsylvania
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
