Job Summary
Conduct home health visits to clients in designated geographic territories in Lancaster County, following designated care plans and administering medications, wound care, and treatments. Perform client assessments, monitor conditions, and report changes to the Clinical or Client Services Manager while executing and documenting doctors' orders. Manage case coordination, accurately record observations and interventions using a touch pad tablet, and maintain independence with effective time management. Requires a current Pennsylvania RN license, one year of recent clinical experience, and strong computer skills with EMR proficiency.
Required Qualifications
- A current license as a Registered Nurse in Pennsylvania
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
