Job Summary
Perform Start of Care, Resumption of Care, recertification, and routine skilled nursing visits to patients in the San Fernando Valley. Conduct comprehensive patient assessments, develop individualized care plans, and complete accurate documentation within 48 hours. Perform medication reconciliation at each visit, provide patient and family education, and collaborate with Care Coordinators and the interdisciplinary team. Maintain compliance with Joint Commission, CDPH, and agency standards. This per diem role supports All Care's commitment to caring for patients in their homes.
Required Qualifications
- At least one year of nursing experience
- Home Health or similar environment experience
- Strong clinical skills
- Organizational skills
- Communication skills
- Ability to work independently
- Time management ability
- Active California RN License
- Current CPR
- Driver's License
- Car Insurance
- Social Security Number
- Current Physical Exam
- TB Test
- Immunization Records (Flu, Hep B, TDAP, MMR and Varicella)
Desired Qualifications
- Knowledge in OASIS documentation
- MyUnity EMR experience
- Bilingual ability (Spanish, Vietnamese, Chinese)
