Registered Nurse

RN provides and coordinates skilled nursing care in patients' homes; completes comprehensive and ongoing patient assessments (SOC, recertification, reassessment), develops and updates individualized care plans, ensures care aligned with physician orders, Medicare Conditions of Participation, and agency policies. Serves on the interdisciplinary team, supports supervision of LPNs and CNAs, coordinates care with physicians and other providers, monitors patient status, communicates changes promptly, and maintains accurate documentation in the EMR to support medical necessity and compliance. Role involves travel to patient homes within assigned territory and requires strong clinical judgment, exceptional assessment and communication skills, and the ability to work independently in a home health setting.
