Job Summary
Conduct home health visits to clients in designated territories throughout Chester County while following designated care plans. Perform assigned duties including medication administration, wound care, treatments, and procedures, and monitor client conditions by reporting changes to the Clinical or Client Services Manager. Execute and document doctors' orders, perform client assessments, and manage case coordination using a touch pad tablet. Maintain accurate documentation of observations, interventions, and evaluations. Requires a current Pennsylvania RN license, one year of recent clinical experience, and ability to travel to cases as assigned. Flexible schedules available Monday through Friday between 8:30am and 5pm.
Required Qualifications
- A current license as a Registered Nurse in Pennsylvania
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
- Must be available at least two days Monday - Friday between the hours of 8:30am-5pm
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
