Job Summary
Provide in-home nursing care across Santa Clara County and surrounding areas, conducting comprehensive patient assessments, developing individualized care plans, and completing Oasis documentation within 24 hours of visits. Collaborate with interdisciplinary teams to coordinate care, monitor patient progress toward goals, educate patients and families about treatment options and resources, advocate for patients’ rights, and maintain accurate EMR documentation. Requires Oasis Home Health experience, RN licensure, and a BSN or related field; 2+ years healthcare experience; ability to work independently and as part of a team; strong assessment, communication, and problem-solving skills; knowledge of community resources for referrals.
Required Qualifications
- RN license in California
- Bachelor's degree in nursing (BSN) or related field
- OASIS experience
- 2+ years in healthcare setting
- Current licensure or certification (RN)
- Ability to work independently and in a team
- Strong assessment, communication, and problem-solving skills
- Knowledge of community resources and services for patient referrals
- Proficiency in electronic medical records (EMR) systems
