Registered Nurse (Home Health) - San Diego

Conduct home health visits to administer services such as IVs, wound care, and assessments while maintaining patient independence. Perform comprehensive initial and ongoing assessments, document findings in Synergy Software, and update physicians regarding patient health changes. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise care plans, including managing transfers, discharges, and re-certifications. Supervise Home Health Aides and coordinate with other team members like PT, OT, and ST as needed. Maintain current CA RN licensure, BLS certification, and IV/Phlebotomy credentials to ensure regulatory compliance. Work a flexible schedule covering seven days a week, including weekends and holidays, with pay ranging from $50 to $140 per visit.
