Job Summary
Conduct comprehensive nursing assessments covering patients' physical, emotional, and psychosocial needs, then build and adjust individualized care plans. Perform hands-on skilled care including wound care, IV therapy, and symptom management while providing education and support to patients and families. Coordinate directly with physicians, therapists, and social workers on care plans, and supervise LPNs/LVNs and home health aides. Document care, schedule appointments, and maintain alignment with agency policy and regulatory standards. Work Monday through Friday, 8:00 a.m. to 5:00 p.m., PRN, with some weeks including on-call rotation, admissions, or urgent response.
Required Qualifications
- Graduate of an approved nursing program
- Current, active RN licensure in the state
- Minimum of one year of experience as a Registered Nurse
- Current CPR certification
- Valid driver's license
- Properly insured vehicle in good working order
- Ability to travel to all business locations
