Registered Nurse Home Health - PRN

Support patient recovery and ability to stay at home by educating families on disease management and providing clinical oversight to the interdisciplinary team. Case manage care for patients with advanced illnesses in partnership with health systems and long-term care partners. Maintain RN licensure in the state of employment and hold current CPR and BCLS certification. Two years of nursing experience required, with home health experience preferred. Current driver's license is mandatory. Flexible scheduling available for part-time and full-time positions.
