Job Summary
Provide home health nursing care to patients in their homes, including educating patients and families and assisting with disease management. Drive care plans forward with the interdisciplinary team, perform case management and provide clinical oversight to ensure quality home-based care. Requires an active RN license in the work state, at least two years of nursing experience (home health experience preferred), a valid driver’s license, and current CPR/BCLS certification. Compensation benefits include onboarding and mileage reimbursement, with a flexible Part-Time schedule.
