Job Summary
Provide skilled nursing care in home health for older adults on private duty, including wound care, IV therapy, and monitoring of chronic/acute conditions. Manage and monitor patients with complex needs (e.g., ventilators, tracheostomies, feeding tubes). Recognize early signs of deterioration and intervene appropriately. Educate patients and family members on disease management, medications, and equipment use. Coordinate care with physicians, therapists, and other healthcare providers. Maintain accurate and timely documentation in electronic health records and ensure compliance with home health regulations and infection control practices.
Required Qualifications
- Current, unrestricted California registered nurse license
- 3 years of experience as a registered nurse
- Home Health experience preferred
- Proof of Negative TB result
- Physical Screening Documentation within the last year
- Active California Drivers license and car insurance
- Cell phone with Internet Access
