Registered Nurse Home Health Part Time PRN

Deliver individualized, one-on-one nursing care in the home to promote patient recovery, safety, and independence. Conduct comprehensive in-home assessments evaluating physical, emotional, and environmental needs, then translate findings into patient-centered care plans. Provide skilled interventions including wound care, infusions, medication management, and chronic disease management tailored to each patient's goals. Educate patients and caregivers on symptom management and daily care, while monitoring progress to identify condition changes early. Coordinate care across providers to ensure continuity and adjust plans to prevent hospitalizations. Complete timely, accurate documentation reflecting the full clinical picture. Requires an active RN license with one year of experience, CPR certification, and reliable transportation. Part-time PRN schedule.
