Job Summary
Deliver professional nursing care in the home setting following physician orders, assessing, planning, implementing, and documenting patient outcomes. Develop individualized care plans with measurable goals, monitor patient progress, and communicate changes to physicians and the interdisciplinary team. Educate patients and caregivers on treatments and medications while performing procedures in compliance with nursing standards. Assess home safety, report concerns, and complete accurate OASIS documentation per Medicare and Medicaid guidelines. Work a standard Monday–Friday schedule (8:00 AM – 4:30 PM) with optional weekend call rotation, requiring reliable transportation and strong time management. Support quality improvement initiatives and collaborate with physical therapy, occupational therapy, and social work staff.
Required Qualifications
- Graduate of an accredited nursing program
- Current RN licensure
- Valid driver's license
- CPR certification
- Reliable transportation
- Must be able to lift 50 lbs
- Must be available for weekend shifts
- Must be available for on-call rotation, weekends, and holidays
- Must be able to work a standard Monday–Friday schedule (8:00 AM – 4:30 PM)
- Must be able to work within a 50-mile radius of Fort Smith, Arkansas
- Must be able to work within the state of Arkansas
- Must be able to perform detailed electronic documentation
- Must be comfortable with digital systems
- Must be organized
- Must be adaptable
- Must be confident working independently in the field
Desired Qualifications
- 1+ year of relevant nursing experience (home health or related field)
