Registered Nurse - Home Health (Long Beach)

Job Summary
Perform Start of Care, Resumption of Care, Recertification, and routine skilled nursing visits for patients in Long Beach and surrounding South Bay areas. Conduct comprehensive patient assessments, develop individualized care plans, and perform medication reconciliation at each visit. Complete accurate and timely documentation within 48-hour compliance windows while providing patient and family education based on diagnosis and care needs. Collaborate with Care Coordinators and the interdisciplinary team to maintain compliance with Joint Commission, CDPH, and agency standards. This per diem field-based role offers flexible scheduling for a Joint Commission Accredited home health agency serving the Los Angeles and Orange County regions.
Required Qualifications
- At least one year of nursing experience
- Home Health or similar environment experience
- Strong clinical skills
- Strong organizational skills
- Strong communication skills
- Ability to work independently
- Time management ability
- Active California RN License
- Current CPR certification
- Driver's License
- Car Insurance
- Social Security Number
- Current Physical Exam
- TB Test
- Immunization Records (Flu, Hep B, TDAP, MMR and Varicella)
Desired Qualifications
- Knowledge in OASIS documentation
- MyUnity EMR system experience
