Job Summary
Provide in-home nursing care to geriatric patients under designated care plans through home visits in Albuquerque and surrounding areas; perform medications administration, wound care, treatments, and procedures; assess patients and coordinate care; document observations and interventions in the EMR; report changes to the Clinical or Client Services Manager and follow doctors' orders; maintain and update care plans and ensure quality home health care; RN license and valid driver’s license required; 1+ year of clinical experience preferred; knowledge of HCHB EMR desired.
Required Qualifications
- Current State or Compact RN license
- Valid state driver’s license and automobile liability insurance
- A minimum of one year of clinical experience is preferred
- Knowledge of HCHB EMR is desired
