Job Summary
RN Case Manager provides skilled nursing care in the home, develops and manages individualized plans of care, coordinates with physicians and interdisciplinary teams, educates and supports patients and families, and completes accurate clinical documentation. Requires an active Idaho (or multi-state) RN license; Home Health/Home Care experience and HCHB EMR, OASIS knowledge are preferred. Boise-based role offering one-on-one in-home patient care with autonomy and strong team support; emphasis on compassionate, patient-centered care and collaboration with physicians and interdisciplinary teams.
Required Qualifications
- Active RN license in Idaho or multi-state license
- Home Health experience preferred (not required)
- Home Care Home Base (HCHB) EMR System knowledge
- OASIS knowledge and experience
- Strong communication and time-management skills
- Compassionate, patient-centered approach
- Valid driver’s license and reliable transportation
