Provide and coordinate care for assigned patients in their homes, developing nursing care plans including Start of Care and admissions as assigned. Support patient recovery and enable them to spend quality time with loved ones by educating families on disease management. Conduct case management and provide clinical oversight to interdisciplinary teams while ensuring physician orders are transcribed appropriately per policy. Assess patient status on an ongoing basis, document and complete charting for all visits within 24 hours, and notify physicians of significant status changes. Cover territories in Madison and Delaware County on a 40-hour week, Mon-Fri, 8a–5p, with on-call for 3-4 weekdays per month.
