Job Summary
Registered Nurse-Home Health provides planning, coordinating and documenting care for an assigned caseload under the supervision of the Case Manager, developing home health plans based on physicians’ orders while ensuring compliance with accrediting standards, federal/state regulations, and agency policies. The role involves participation in Quality Assessment and Performance Improvement (QAPI) activities, scheduling full-time (40 hours/week) home health visits in Green Bay and surrounding area, and delivering patient-centered nursing care. Requires an RN license, BLS, and a valid driver’s license to perform home visits; Bachelor’s degree in nursing is preferred; prior home health or hospice experience and acute nursing experience are also preferred.
Required Qualifications
- Graduate from an accredited and approved school of nursing accepted for licensure in the state of practice
- Licensed as a Registered Nurse (RN) in the state of practice
- Wisconsin: RN licenses via Nurse Licensure Compact rules
- Basic Life Support (BLS)
- Valid driver's license, automobile insurance and own transportation for home visits
- Bachelor's Degree in nursing is preferred
