Registered Nurse Home Health

Home Health Registered Nurse (RN) provides patient care and case management in patients’ homes, enabling them to stay at home and engage in daily activities. Responsibilities include case managing and providing clinical oversight to an interdisciplinary team, educating patients and families on disease management, and supporting patient recovery. Requirements include an RN license in the work state, two years of RN experience (home health experience preferred), a valid driver’s license, and CPR/BCLS certification. The role is based in Suffolk, VA with in-person, home-based care expectations.
