Registered Nurse Home Health Franklin NC Team

Assess home care patients identifying physical, psychosocial, and environmental needs through documentation, clinical records, case conferences, and on-site evaluations. Complete OASIS assessments, visit paperwork, and clinical notes while communicating significant findings to the Clinical Manager and physician. Supervise home health aides and LPNs/LVNs to ensure quality and continuity of services. Participate in on-call rotation and emergency calls per agency policy. This role supports CarePartners Health Services, a Mission Health division offering post-acute care including rehabilitation and hospice in western North Carolina.
