Registered Nurse Home Health Franklin NC Team

Assess home care patients identifying physical, psychosocial, and environmental needs through documentation, case conferences, and on-site evaluations. Complete OASIS assessments, visit paperwork, and clinical notes ensuring accurate communication with physicians and interdisciplinary team members. Supervise home health aides and LPNs/LVNs to ensure quality and continuity of services. Communicate significant findings, problems, and changes to the Clinical Manager and physician, documenting all interventions. Participate in on-call rotation and emergency calls according to agency policy. Provide coordinated skilled nursing care to patients of all age groups in the home under Clinical Manager supervision.
