Assess home care patients identifying physical, psychosocial, and environmental needs through documentation, clinical records, case conferences, and on-site evaluations. Collaborate with the interdisciplinary team to establish patient goals, perform in accordance with physician orders, and complete OASIS assessment and visit paperwork. Supervise home health aides and LPNs/LVNs to ensure quality and continuity of services. Communicate significant findings and changes to the Clinical Manager and physician, documenting all interventions. Participate in on-call rotation and emergency calls as required by agency policy.
