Job Summary
Support patient recovery and ability to stay at home by educating families on disease management and providing clinical oversight to the interdisciplinary team. Case manage care for patients with advanced illnesses in partnership with health systems and long-term care partners. Requires an RN license in the state of work, two years of registered nurse experience, and preferred home health experience. Must hold a current driver's license and CPR and BCLS certification.
