Job Summary
Conduct comprehensive patient assessments to determine home care needs and develop individualized plans of care with measurable goals. Administer medications and treatments per physician orders while supervising LVNs and CHHAs according to agency policies. Provide patient and family education on medication management and disease prevention, then coordinate care with all providers and ordering physicians. Document clinical notes accurately and timely, reporting any changes in patient condition to the clinical supervisor. Participate in QAPI programs, professional development, and monthly in-services while ensuring compliance with HIPAA and infection control standards. Requires a licensed Registered Nurse with at least one year of experience, a valid driver's license, and current CPR certification.
Required Qualifications
- Licensed Registered Nurse authorized to practice professional nursing in the state
- Holds and maintains current CPR certification
- Must have a valid and active driver's license
- Must have car insurance
- Must have an automobile in good working condition
- Must have at least one year of experience as a professional nurse, preferably in home health care, hospice or public health
- Must have great interpersonal and communication skills
- Must be proficient in the use of computers for computerized clinical documentation
- Must be capable of autonomous work with basic safety knowledge
- Graduate of an accredited school of nursing
