Job Summary
Registered Nurse Home Health role providing in-home clinical nursing care across patient settings, including assessment, planning, implementing, and evaluating care plans; involves 1:1 patient care, travel within coverage area, and coordination with an interdisciplinary team. Requires current RN license, 1+ year clinical experience, ability to travel up to 50% with a dependable vehicle, and preferred nursing degree or higher; describes responsibility for evaluating patient needs, documenting findings, communicating changes to supervisors and physicians, and delivering comprehensive home-based care.
Required Qualifications
- Current, unrestricted state RN license
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver’s license, and current auto insurance
- Must be able and willing to travel 50%
- Associates Degree or Bachelor’s Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with HomeCare HomeBase is preferred
