Registered Nurse | Home Health

Registered Nurse Home Health role to provide in-home patient care across the Kokomo, IN branch coverage area. You will perform initial evaluations, plan, implement and evaluate the plan of care, conduct follow-up visits, and reassess patient needs. Responsibilities include communicating significant changes to the Clinical Supervisor and other team members, maintaining HIPAA privacy, documenting care in clinical records and Interdisciplinary Team reports, and coordinating with the care team. Travel up to 50% within the coverage area is required; must have a dependable vehicle, valid driver's license, and current auto insurance. Associates Degree or Bachelor’s Degree in Nursing is preferred; 1+ year clinical nursing experience and prior Home Health/Hospice experience and HomeCare HomeBase familiarity are preferred.
