Job Summary
Assess assigned case loads of home health patients to identify physical, psychosocial, and environmental needs, then develop and implement individualized plans of care. Conduct initial evaluations and follow-up visits, revising treatment plans based on changing patient conditions and communicating significant findings to clinical supervisors, physicians, and facilities. Maintain patient and staff privacy in accordance with HIPAA regulations while ensuring all activities align with board and leadership vision. Requires current unrestricted state RN license, one year of clinical experience, and ability to travel 50% within the coverage area. Preferred qualifications include a bachelor's degree, prior home health or hospice experience, and familiarity with HomeCare HomeBase.
Required Qualifications
- Current, unrestricted state RN licensee
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
- Must be able and willing to travel 50%
Desired Qualifications
- Associates Degree or Bachelor's Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with HomeCare HomeBase is preferred
