Job Summary
Conduct home visits to clients in Kent County, DE, administering medications, wound care, and treatments while following designated care plans. Perform client assessments, monitor conditions, and report changes to managers, then accurately document observations, interventions, and evaluations using a touch pad tablet. Manage case coordination and execute doctors' orders for adult patients in Smyrna, Clayton, and Dover. Requires a current DE RN license, one year of clinical experience, and strong EMR skills.
Required Qualifications
- A current license as a Registered Nurse in DE
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
