Job Summary
Conduct home health visits in Windham and Windsor County, Vermont, following designated care plans and administering medications, wound care, and treatments. Perform client assessments, monitor conditions, and report changes to managers while accurately documenting observations, interventions, and evaluations on a touch pad tablet. Manage case coordination and execute doctor's orders within assigned geographic territories. Requires a current Vermont RN license, one year of clinical experience, and ability to travel.
Required Qualifications
- A current license as a Registered Nurse in Vermont
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
