Registered Nurse Home Health

Provide direct patient care including triage, blood draws, wound care, and execution of physician orders while monitoring and documenting conditions. Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team. Coordinate care across disciplines and support discharge planning to ensure continuity. Educate patients, caregivers, and staff while participating in team and care plan meetings. Participate in on-call rotation and travel to patient residences or care facilities. Requires Registered Nurse licensure from the Illinois Board of Nursing, BLS certification, and a valid Illinois driver's license. Two years of hospital or home health experience is strongly preferred.
