Job Summary
Conduct comprehensive initial and ongoing assessments of patients and families to determine hospice needs, utilizing the nursing process to establish care plans and goals. Administer prescribed medications and treatments while providing health care instructions, counseling, and preventive nursing procedures. Coordinate patient care as a Case Manager by communicating with physicians, community health partners, and the interdisciplinary group to ensure continuity. Complete accurate clinical notes, participate in on-call duties, and supervise ancillary personnel as required.
Required Qualifications
- Graduate of National League for Nursing accredited school of nursing
- Registered nurse with current licensure to practice professional nursing in the state
- Demonstrates excellent observation, problem solving, verbal and written communications; nursing skills per competency checklist
- Shows ability to organize and prioritize workload independently
- Prolonged or considerable walking or standing
- Able to lift, position, and/or transfer patients
- Able to lift supplies and equipment
- Considerable reaching, stooping, bending, kneeling, and/or crouching
- Visual acuity and hearing to perform required nursing skills
- Maintains a current CPR certification
- The ability to drive and/or have access to transportation for agency related travel
- Must have and maintain a valid driver's license
- Maintain automobile insurance coverage
- Have access to an automobile
- Self-directed and able to work with minimal supervision
Desired Qualifications
- Minimum of two years of experience, at least one of which is in the area of public health, home care, or hospice nursing
