Job Summary
Perform Start of Care, Resumption of Care, Recertification, and routine skilled nursing visits to patients in Southeast LA and surrounding areas. Conduct comprehensive patient assessments, develop individualized care plans, and perform medication reconciliation at each visit. Complete accurate documentation within 48 hours and provide patient and family education based on diagnosis and care needs. Collaborate with Care Coordinators and the interdisciplinary team while maintaining compliance with Joint Commission, CDPH, and agency standards. This per diem role offers flexible scheduling and competitive pay for a California-licensed RN with strong clinical judgment.
Required Qualifications
- At least one year of nursing experience
- Home Health or similar environment experience
- Strong clinical skills
- Organizational skills
- Communication skills
- Ability to work independently
- Time management ability
- Active California RN License
- Current CPR
- Driver's License
- Car Insurance
- Social Security Number
- Current Physical Exam
- TB Test
- Immunization Records (Flu, Hep B, TDAP, MMR and Varicella)
Desired Qualifications
- Knowledge in OASIS documentation
- MyUnity EMR experience
- Bilingual ability (Spanish, Vietnamese, Chinese)
