Job Summary
Conduct Start of Care, Resumption of Care, Recertification, and routine skilled nursing visits for patients in Orange County and surrounding areas. Perform comprehensive patient assessments, develop individualized care plans, and complete accurate documentation within 48 hours. Reconcile medications at each visit, provide patient and family education, and coordinate care with physicians and interdisciplinary teams. Ensure electronic visit verification compliance and maintain adherence to Joint Commission, CDPH, and agency standards. Requires active California RN license, CPR certification, valid driver's license, and ability to work independently.
Required Qualifications

- Current CPR certification
- Valid Driver's License

- Ability to manage time efficiently
- Current Physical Exam
- TB Test/Clearance
- Immunization Records (Flu, Hep B, TDAP, MMR and Varicella)
- Compliance with all credentialing requirements
- Ability to meet documentation and submission deadlines
