Job Summary
Conduct start-of-care admissions for home health visits across Dauphin and Cumberland counties by following designated care plans, making home visits, and performing assigned duties including medication administration, wound care, and treatments. Monitor client conditions, report changes to managers, and accurately document observations, interventions, and evaluations using a touch pad tablet while executing and tracking doctors' orders. Manage case coordination and client assessments as necessary, maintaining independent time management and strong interpersonal skills. Requires a current Pennsylvania RN license with one year of recent clinical experience and prior home care background.
Required Qualifications
- A current license as a Registered Nurse in Pennsylvania
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
