Educate patients and families on disease processes while promoting health and independence through teaching and rehabilitative measures. Manage and monitor patient conditions by performing comprehensive assessments, developing individualized care plans per physician orders, and collaborating with the multidisciplinary team including PT, OT, ST, MSW, and paraprofessionals. Complete timely and accurate documentation, including OASIS assessments, admissions, re-certifications, and care coordination. Communicate changes in care plans, lab results, and referrals to maintain continuity, engage care team members, and participate in case conferences. Maintain ongoing communication with the clinical manager regarding assigned caseload progress. This direct-to-hire, permanent position offers full-time, part-time, or PRN options with no shifts, allowing nurses to create their own schedule. Located in the Staunton, VA area, candidates must hold current VA RN licensure, a recent TB test or chest X-ray, and valid CPR certification.
