Job Summary
Provide skilled nursing care onsite in patients' homes, including administering medications, performing wound care, and conducting assessments. Develop and implement individualized care plans in collaboration with physicians, monitor patient conditions, and report progress to the Clinical Manager. Educate patients and families on disease management while maintaining accurate records and overseeing Home Health Aide documentation. Requires Med surg, ICU, or ER experience, current RN licensure, and CPR certification. Valid driver's license and auto insurance are mandatory. Weekly schedule is 32 hours with a weekend rotation from Friday through Monday.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license
- Auto insurance

- TB screening
