Job Summary
CenterWell Home Health RN - provide skilled nursing care in patients' homes: administer medications, wound care, and patient assessments; develop and implement individualized care plans with physicians; monitor patient conditions and report changes; educate patients and families on disease management, medications, and treatment options; maintain accurate care records and coordinate with healthcare professionals; oversee Home Health Aides and ensure documentation completion; contribute to patient recovery and independence; $10K sign-on bonus; 40-hour work week; TB screening and driver-safety program requirements.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license, auto insurance and reliable transportation
