Job Summary
Deliver comprehensive skilled nursing care to patients in their homes, assisted living facilities, or nursing homes while managing a caseload from admission through discharge. Conduct daily assessments, develop individualized care plans, and coordinate services with physicians and an interdisciplinary team to support recovery and chronic disease management. Maintain proficiency in OASIS documentation and care planning while adhering to Medicare, Medicaid, and third-party payor requirements. Requires current State RN license, minimum one year of clinical experience, and daily travel within the Grand Rapids service area. BSN preferred with OASIS certification. Full-time Monday through Friday, 8 a.m. to 5 p.m. with a $10,000 sign-on bonus.
Required Qualifications
- Graduate of an accredited Nursing School: ASN or BSN
- Current State License as a Registered Nurse
- Minimum 1 year of experience as a licensed RN in a clinical care setting
- Reliable transportation, valid driver's license, and valid automobile insurance coverage
- Willingness to travel daily for home visits within service area
Desired Qualifications
- Bachelor of Science in Nursing (BSN) preferred
- Experience in home health environment preferred
- OASIS certification or willingness to obtain preferred
