Registered Nurse, Home Care VHS- Per Diem (Bergen/Passaic/Morris counties)

Assess, develop, implement, and evaluate home nursing care needs for assigned patients in accordance with organization policies and external regulations. Ensure compliance with physician orders, document care progression toward discharge goals, and maintain visit frequency within authorization ranges. Complete nursing assessments, medication reconciliation, and collaborative care planning updates within two weeks of status changes. Act as clinical supervisor for para-professional home health aide staff, conducting documented supervisory visits and participating in staff evaluations. Serve as liaison among physicians, managers, and the healthcare team to coordinate services and manage patient transfers. Identify appropriate payors, maintain authorizations, and report care problems to management with proposed solutions. Adhere to a minimum rate of $45.27 hourly with flexible scheduling opportunities.
