Job Summary
Provide skilled nursing care to patients in their homes under physician orders, including assessments, medication review, wound care, IV therapy, and clinical interventions. Collaborate with the interdisciplinary care team to develop individualized care plans, educate patients and families on disease management and self-care, and maintain accurate documentation. Communicate patient status changes and coordinate care with appropriate community resources while supporting quality and safety initiatives. Schedule: Monday–Friday, 8:00 AM–4:30 PM with 1–2 on-call days monthly and 3 holidays. Sign-on bonus up to $15,000. New York State Licensed RN with one year of direct patient care experience required.
