Registered Nurse - Home Care/Long Term Care Admissions RN

Perform initial and ongoing assessments of patients and families, documenting findings per TriHealth guidelines and communicating results to physicians. Plan and individualize care based on assessment data, developing short- and long-term goals with patient input while supervising delegated tasks. Administer medications and treatments safely, monitor effects, and evaluate care plans to ensure optimal outcomes. Provide health counseling, educate staff on specialty skills, and maintain a therapeutic environment that respects patient autonomy and dignity. Complete patient assignments and documentation within scheduled timeframes while recognizing emergency situations. Full-time evening shift (32 hours weekly, Tuesday–Friday, 3 p.m.–11:30 p.m. with rotating third weekends). Mileage reimbursement available for field-based travel.
