Job Summary
Provide quality nursing services to Clarity Care clients by performing assessments, admissions, discharges, and developing individualized plans of care. Secure physician orders, document all consultations and treatments in client records, and promptly notify medical personnel of significant condition changes. Supervise and instruct personal care workers and residential employees, evaluating their competency and developing time and service reporting mechanisms. Participate in the Occupational Health program by screening new employees and administering TB tests. Take accurate notes of all treatments and record patient care services in department records. Work one-on-one with clients to promote independence, establish care goals, and teach self-care skills. Join the on-call rotation as directed and travel to remote locations as needed.
Required Qualifications
- Current licensure as a registered nurse under s.441.06, Wis. Stats.
- Training and experience in the provision of personal care services or in a related program.
- Participate in on call rotation as directed.
- Registered Nurse - Casual needs to sign all of the documents for PC agency requirement
- Undergo successful orientation and participate in competency-based assessments of skills on a regular basis as required by state law.
