Job Summary
Meet new patients, conduct start-of-care assessments, and develop individualized plans of care addressing medical needs and family involvement. Track patient progress, adjust care plans, and report condition changes to physicians. Instruct caregivers on care plan details, review documentation, and perform supervisory visits to ensure compliance and quality. Lead training sessions on safety protocols and clinical standards while maintaining relationships with referral sources and community partners. Ensure adherence to OSHA, DMAS, and state licensure regulations. Work Monday through Friday with no weekends, holidays, or on-call shifts, with mileage reimbursement for home visits. Required Qualifications
- Active RN license in Virginia (compact/multi-state accepted)
- Minimum of 1 year in a nursing role (LPN-to-RN pathway acceptable)
- Associate's Degree in Nursing or higher
- Strong communication and presentation skills
- Ability to work collaboratively across teams
- Reliable transportation for home visits (mileage reimbursed)
- Comfortable with technology (computer/iPad documentation)
- Willingness to travel regularly within assigned area
