Job Summary
As a Home Care Supervisor, lead clinical care and mentor patients, caregivers, and staff in the Lexington area. Responsibilities include onboarding new patients, developing individualized care plans, monitoring patient progress, classroom and on-the-job training for staff, supervising caregivers and ensuring quality, regulatory compliance, and community partnerships. Requires an active SC RN license, an Associate's Degree in Nursing or higher, minimum 1 year nursing experience, and willingness to travel within the assigned area for home visits (mileage reimbursed). The role offers a full-time schedule with a Monday–Friday footprint and no weekends, holidays, or on-call shifts.
Required Qualifications
- Active RN license in South Carolina
- Associate's Degree in Nursing or higher
- Minimum of 1 year in a nursing role
- Strong communication and presentation skills
- Ability to work collaboratively across teams
- Willingness to travel regularly within assigned area
