Job Summary
Meet new patients, complete start-of-care assessments, and develop individualized plans of care addressing medical needs and family involvement. Track patient progress, adjust care plans, and report condition changes to physicians. Instruct caregivers on care details and best practices, review documentation for accountability, and conduct supervisory visits to ensure compliance. Teach household members assisting with patient care, lead training sessions on safety protocols, and ensure adherence to OSHA, DMAS, and CCC Plus regulations. Prepare clinical notes, coordinate progress reports, participate in interdisciplinary meetings, and build relationships with referral sources and community partners. Maintain active RN license in Virginia with at least one year of nursing experience. Monday–Friday schedule with no weekends, holidays, or on-call; competitive wages and mileage reimbursement available.
Required Qualifications
- Active RN license in Virginia (compact/multi-state accepted)
- Minimum of 1 year in a nursing role (LPN-to-RN pathway acceptable)
- Associate's Degree in Nursing or higher
- Strong communication and presentation skills
- Ability to work collaboratively across teams
- Reliable transportation for home visits (mileage reimbursed)
- Comfortable with technology (computer/iPad documentation)
- Willingness to travel regularly within assigned area
