Job Summary
Coordinate healthcare services for 48 single adults with mental health and/or substance use issues by assisting residents in managing medications and navigating appointments and referrals. Provide health education on chronic disease management and preventative strategies while supporting acute care coordination. Deliver staff training on health and mental health issues. Report to the Executive Director within our newly constructed supportive housing program in Brooklyn. Licensed Registered Nurses with five years' experience working with special needs populations are required.
