Job Summary
Registered Nurse – Heart Failure Coordinator role overseeing patient-centered cardiac care within the Cardiovascular/Heart Failure units. Responsibilities include assessing patients’ physical and psychosocial conditions, developing and documenting individualized plans of care, coordinating implementation with the healthcare team, evaluating patient response and readiness for discharge, and providing patient/family education. The RN may serve as preceptor to new staff, act as a charge nurse, and support ongoing professional development and adherence to unit standards. Cardiac/Neuro care skills emphasized, including neuro assessment, airway management, care for trached patients, post-surgical heart failure and LVAD/ECMO considerations, and familiarity with the unit’s teaching program.
Required Qualifications
- Graduation from an accredited school of nursing.
- BSN preferred.
- BCLS certification current or must obtain within one month of hire.
- Licensed and currently registered nurse in the State of Connecticut.
- Clinical specialty certification highly encouraged.
