Job Summary
Registered Nurse – Heart Failure Coordinator responsible for assessing the patient’s physical and psychosocial condition, developing and documenting a Plan of Care with patient/family involvement, coordinating care with the healthcare team, and evaluating outcomes. Performs precepting of new staff and charge nurse duties; demonstrates competency in neuro assessment and airway management, including care of trached patients and ventilated patients. Involves education planning for discharge and follows the facility’s teaching program (Tia). Opportunities to participate in various cardiac ICU/CTICU/Stepdown units and collaborate with interdisciplinary teams to ensure high-quality care.
Required Qualifications
- Graduation from an accredited school of nursing.
- BSN preferred.
- Licensed and currently registered nurse in the State of Connecticut.
- Current BCLS certification or must obtain within one month of hire.
- Clinical specialty certification highly encouraged.
